2型糖尿病患者即刻种植骨愈合情况的影像学分析

Imageological analysis of immediate implant healing in patients with type 2 diabetes mellitus

摘要: 目的: 比较2型糖尿病患者和对照组即刻种植骨愈合情况的影像学差异。方法: 选择10例糖尿病需即刻种植患者,10例无糖尿病患者为对照,在围术期控制血糖在正常范围条件下,进行单颗牙即刻种植。结合CBCT观察糖尿病组和对照组术后即刻和术后三个月种植体颈部、中部、根尖部骨密度差值以及植体周围透射区宽度变化。结果: 糖尿病和对照组种植体颈部、中部、根尖部骨密度差值无统计学意义(F=1.37,P=0.24>0.05)。种植体周围透射区宽度方差分析结果显示,(F= 21.86,P<0.01),有统计学意义。糖尿病组颈部的透射区宽度差值均值(1.27±1.34mm)较无糖尿病对照组(0.51±0.47mm)高。结论: 围术期控制血糖在正常范围情况下,影像学上2型糖尿病患者即刻种植植体周围骨密度差值和对照组无差别,但糖尿病组植体颈部新骨形成较慢,远期效果还有待进一步观察。

Abstract: Objective: To compare the imaging differences of immediate implant healing between type 2 diabetes mellitus and control group. Methods: Ten patients with diabetes mellitus and ten patients without diabetes mellitus were selected as controls. Under the condition of controlling blood sugar in normal range during perioperative period, single tooth was implanted immediately. CBCT was used to observe the difference of bone mineral density (BMD) in the neck, middle and apex of implant and the width of transmission area around implant in diabetic group and control group immediately and three months after operation. Results: There was no significant difference in bone mineral density between diabetes mellitus group and control group(F=1.37, P=0.24>0.05). The results of variance analysis of transmissive zone width around implants showed that there was statistical significance(F=21.86, P<0.01). The mean width of the transmission zone in the neck of the diabetic group(1.27+1.34mm) was higher than that of the non-diabetic control group(0.51+0.47mm). Conclusion: The bone mineral density around immediate implants in type 2 diabetes mellitus patients was similar to that in control group when blood sugar was controlled in normal range during perioperative period. However, the formation of new bone in neck of implants in type 2 diabetes mellitus patients was slow, and the long-term effect needed to be further observed.
