美学区牙槽骨解剖特征对引导骨再生效果的影响

摘要: 美学区种植常需联合应用骨增量手术,以获得充足的种植体周骨量,同时形成良好的轮廓外形并为软组织美学提供基础。引导骨再生(guided bone regeneration,GBR)是最常用的美学区骨增量技术,其临床疗效确定。大量文献讨论了影响GBR术后效果的临床因素,包括患者相关因素及手术相关因素。本文聚焦于患者相关的局部因素,在牙槽骨解剖特征这一范畴内回顾相关文献证据,分别从骨缺损形态、种植位点初始骨量、种植位点牙槽骨血供,以及即刻种植拔牙窝特征等几方面总结了这些因素与GBR术后效果之间的关系,以期为美学区骨缺损患者的治疗方案设计、GBR骨增量的预后判断提供理论依据和参考。

Abstract: The implantation in the aesthetic zone is often accompanied by bone augmentation surgery to acquire enough peri-implant bone volume. Meanwhile, bone augmentation may achieve greater contour morphology and lay a solid foundation for soft tissue aesthetic effects. The guided bone regeneration (GBR) is the most common bone augmentation surgery performed in the aesthetic zone and has a satisfactory clinical outcome. Considerable literatures have discussed the factors influencing the postoperative effects of GBR, including factors associated with patients and surgery. This review focuses on the patient-related local factors, summarizing the evidence about the anatomical characteristics of the alveolar bone. We discuss the relationship between the anatomical characteristics of the alveolar bone and postoperative outcome of GBR, including the morphology of alveolar bone defect, the initial bone volume, the blood supply of alveolar bone at the implant sites, and the characteristics of the extraction socket in immediate implant placement, to provide a reference for the treatment planning for patients with bone defects in the aesthetic area and for the long-term prognosis of GBR.
