牙周病患者拔牙后即刻种植和延期种植临床效果的Meta分析

摘要： 目的: 研究牙周病患者拔牙后即刻种植与延期种植的临床效果。方法: 检索PubMed、Cochrane Library和CNKI等6个数据库,时间截止于2019年12月,两位作者独立完成文献筛选、数据提取和偏倚风险评估。使用RevMan5.2软件进行资料的提取和统计学分析。结果: 共纳入8项研究,Meta分析显示,种植术后第3、6、12个月的红色美学评分即刻种植组高于延期种植组,差异有统计学意义;术后第3个月的种植体颈部骨吸收量即刻种植组低于延期种植组,差异有统计学意义;探诊深度及种植成功率两组无统计学差异。结论: 即刻种植与延期种植可获得同样的远期治疗效果。短期内即刻种植比延期种植美学效果更好。需要更大样本更长观察时间的随机对照实验以获得更可靠的结果。

Abstract: Objective: To study the clinical effect of immediate and delayed implantation on patients with periodontal disease after tooth extraction. Methods: Six databases including PubMed, Cochrane Library, and CNKI were searched up to December 2019. The two authors independently performed literature screening, data extraction, and bias risk assessment. Data were extracted and statistically analyzed using RevMan5.2 software. Results: A total of 8 studies were included. The meta-analysis showed that the red aesthetic score at 3, 6 and 12 months after implantation was higher in the immediate implant group than the delayed implant group. The amount of bone resorption in the neck of the immediate implantation group was lower than that of the delayed implantation group, and the difference was statistically significant; there was no statistical difference in the depth of exploration and the success rate of implantation between the two groups. Conclusion: The similar long-term treatment effect can be obtained with immediate and delayed implanting. Immediate implanting in the short term has better results than delayed implanting. Randomized controlled experiments requiring larger samples and longer observation times for more reliable results.
